Minor (Under 18) Release of Liability for Participation in Gymnastics. Form must be initialled and signed by the guardian of the participant.

Adult Waiver

Release of Liability Form

Adult (18+) Release of Liability for Participation in Gymnastics. Form must be initialled and signed by the adult participant.

 

PIPA Form​

Adult and Child PIPA Form
